G Suite for Education Parent Permission Form

Dear Parents/Guardians,

The De Anza College Academy utilizes G Suite for Education for students, teachers and staff. Using these tools, students collaboratively create, edit and share files and websites for Academy related projects and communication with students and teachers.

De Anza College Academy does not create and manage student accounts or e-mail addresses. Parents should sign students up for access to Academy G Suite courses from home, using the personal e-mail address of the parents’ choice. This permissionform describes the tools and student responsibilities for using Google services. The following services are provided in G Suite for Education for the De Anza College Academy:
Classroom – a blended learning platform for schools that aims to simplify creating, distributing and grading assignments in a paperless way
Drive and Docs – file management, word processing, spreadsheet, drawing and presentation tools similar to Microsoft Office
Calendar - a calendar providing the ability to organize schedules, daily activities and assignments
Google Chrome Sync – a feature that allows end users to synchronize bookmarks, history, passwords and other settings across all devices where they are signed into Chrome
Sites – a website creation tool through which students can create, edit and share both files and websites for school related projects
Google Hangouts and Hangouts Chat – a unified communication service that allows students to initiate and participate in text, voice or video chats, either one on-one or in a group
Google Vault – an administrators’ tool for archiving and eDiscovery of class content and communication
Jamboard Service – an interactive and shareable digital whiteboard
Keep – a note-taking tool for creating lists, notes and to-do items that syncs with Drive
Blogger – a free online tool for creating a basic webpage containing written works
Google Earth – a program that renders a 3D representation of Earth based on satellite imagery
Google Groups – a tool that allows students and teachers to create and participate in online forums, share documents and collaborate on group projects
Google Maps – a web-based service that provides detailed information about geographical regions and sites around the world
These services are hosted by Google on their servers and are available at any time fro any Internet-enabled device. Some examples of student use could include, but are not limited to, class projects, electronic portfolios and working in groups on presentations to share with others.

Foothill-De Anza Community College District User Responsibilities for Computer and Network Use, including G Suite for Education (AP3250) The Board recognizes that computers and networks can provide access to resources on and off campus, as well as the ability to communicate with other users worldwide. Such open access is a privilege and requires that individual users act responsibly. Users must respect the rights of other users, respect the integrity of the systems and related physical resources and observe all relevant law, regulations and contractual obligations.

For District employees, the intended uses of the District Network are those which are reasonable and necessary for the pursuit of job duties; for students, the intended uses are those which are reasonable and necessary for the pursuit of instructional activities. Although personal use is not an intended use, the District recognizes that the Network will be used for incidental personal activities provided that such use is within reason and provided that such usage is ordinarily on an employee’s own time, is occasional, and does not interfere with or burden the District’s operation.

“Unauthorized uses” include prohibited uses and any other use for a prohibited purpose, including illegal activities, messages which may constitute discrimination or harassment under state or federal law, or anything that interferes with the intended use. These types of prohibited uses and purposes are further defined in Administrative Procedure 3250.

All users of the District Network must read, understand, and comply with this Policy as well as Administrative Procedures 3250, and any additional guidelines established by the District. Such guidelines will be reviewed by the District and may become subject to Board approval as a District policy or procedure. By using any part of the District Network, users agree that they will comply with this Policy.

De Anza College Academy Technology Use and Federal Law
Children's Online Privacy Protection Act (COPPA) COPPA applies to commercial companies and limits their ability to collect personal information from children under 13. By default, responsive advertising is turned off for De Anza College Academy’s G-Suite applications. This permission form allows the school to act as an agent for parents in the collection of information within the school context. The school's use of student information is solely for education purposes.

Family Educational Rights and Privacy Act (FERPA) FERPA protects the privacy of student education records and gives parents the rights to review student records. Signing this form acknowledges that some student records, such as portfolios of student work with teacher feedback, may be stored on Google servers.

By enrolling your student in the De Anza College Academy classes, you confirm that you have read and understand the following:

Under FERPA, a student's education records are protected from disclosure to third parties. You understand that information that could be considered part of your student's education records may be stored in G Suite for Education and by virtue of this online
environment may be accessible to someone other than your student and the De Anza College Academy.

You allow your student's education record to be stored by Google and understand that by participating in G Suite for Education, information about your child will be collected and stored electronically. You understand that you may ask for
your child's account to be removed at any time.

You give permission for your child to access the Foothill-De Anza Community College District’s G-Suite account and applications. You agree either to assist your child in joining or give permission for your child to join the FHDA G Suite from home, using the personal e-mail account of your choice. This means your child will have access to: Classroom, Drive and Docs, Calendar, Chrome Sync, Sites, Hangouts and Hangout Chats, Vault, Jamboard Service, Keep, Blogger, Earth, Groups and Maps.
